Cd exploded in drive - does this happen much?

In my dads computer, watching a cd rom lecture, big bang then tray wouldnt eject. Had to take the drive apart - 000's of bits of disc and silver foil in the workings. all hoovered out and seems to be working okay. Does this happen often? I'd imagine it cant always be fixed? is there any way of avoiding it?
